We Discover Transitions
A comprehensive journal to guide young people through navigating their transition from school to post-16 education, employment or training. Working with their school and support network, the guide helps young people to explore their interests, options and support requirements, leading to confident next steps. The journal works particularly well for young people with additional needs and disabilities.
